About The Position
In this role, you will support patient care delivery as part of the home health team, working under the direction of the Clinical Manager. You will assist with patient care activities for a designated group of patients, support the delivery of care aligned with Advocate Health at Home standards, help ensure patients receive high-quality, cost-effective care, promote and support patient and family rights, and contribute to maintaining and improving overall quality outcomes.
Requirements
- Active license in the State of Illinois
- Minimum of 2 years of medical-surgical (med/surg) experience
- Comfortable using computer-based scheduling and documentation systems
- Strong written and verbal communication skills
- Effective interpersonal and teamwork skills
