About The Position
Sage Group HealthCare Staffing is seeking a Home Health LPN for a travel/contract position in Spokane Valley, WA. This role involves providing skilled nursing care under RN supervision, monitoring patient conditions, administering medications, educating patients and families, and completing clinical documentation according to agency protocol and Medicare/Federal guidelines. The LPN will cover territories in Spokane county and possible lower Stevens county.
Requirements
- LPN experience: 1 - 2 years
- Recent/Current Home Health Nursing experience: 1 year required within the last 3 years
- Home Care Home Base (HCHB) charting
- Valid Driver’s License
- Personal Auto Insurance
- LPN License for State of Washington or Compact if applicable
- BLS Certification from AHA or American Red Cross
Responsibilities
- Provide skilled nursing care as ordered by the attending physician under RN's supervision.
- Observe and monitor patient conditions and notify RN case manager of any changes.
- Administer medication as prescribed by the physician under RN's supervision.
- Educate the patient on preventative measures and help decrease re-hospitalizations.
- Educate the patient and family on the disease process using teach back methods.
- Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Understand and follow agency policies, procedures, rules, and regulations.
- Communicate changes in schedule/availability to schedulers or supervisors.
- Attend IDT Team meetings.
