About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health LPN to work with their Home Health team to deliver individualized care to patients in their homes in central Hillsborough county (South Tampa/Carrollwood area). They provide skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support. This PRN role involves 10-15+ home health visits per week, with required weekend rotations.

Requirements

• Active Florida LPN license
• Graduate of an approved nursing program
• Minimum of 1-2 years of experience as a LPN

Nice To Haves

• Home health care experience preferred
• Experience using Homecare Homebase (HCHB) EMR strongly preferred
• Intravenous (IV) Certification strongly preferred

Responsibilities

• Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
• Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Perform patient and family instruction on diet, medication, and treatment.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
