About The Position

Angels of Care Home Health is seeking compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role focuses on delivering patient-centered care according to physician orders and active plans of care, making a significant difference in the lives of patients and their families. The position involves ongoing patient assessments, administration of medications and treatments, care coordination, family education, and implementing preventative initiatives to ensure quality care.

Requirements

• Active RN or LPN/LVN license
• Provide care in a client home setting
• Ability to make a positive and lasting impression!

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
