This PRN (fill-in or as needed) position provides general nursing care under the delegation of the RN in the home setting in accordance with established standards, policies, procedures, licensure, and scope of practice, under the supervision of a registered nurse.

Requirements

  • Graduate of an approved Licensed Practical Nurse Program.
  • One year in Medical/Surgical nursing is required.
  • Holds or is eligible for current licensure in the State of South Dakota.
  • Valid South Dakota driver's license is required.

Responsibilities

  • Utilizes the nursing process in providing patient care in the home setting and includes the process of documentation, identifying unusual conditions, and utilizing initiative and judgment to carry out appropriate responses.
  • Assists in the completion of the admission process and ongoing patient assessment for patients in the home setting.
  • Administers prescribed medications, treatments, and patient-specific cares to the home patient in accordance with approved nursing techniques.
  • Observe the patient, record significant conditions and reactions, and notify the supervisor or Physician of the patient's condition and reaction to drugs, treatments, and significant incidents throughout the progression of care.
  • Participates in patient care via the interdisciplinary team approach from admission to discharge.
  • Follows medical treatment plan through execution of physician orders.
  • Contacts physician or orders to meet patient needs.
  • Participates in a patient education plan according to the individualized needs of the patient, as prescribed by Physician and/or Agency policy including instruction to the patient, family, and others, as identified.
  • Coordinates home care with the Department of Social Services social workers.
  • Completes annual competency skill testing.
  • Assists with departmental orientation of LPN and non-licensed assistant personnel training.
  • Promotes and participates in quality assurance, safety, and infection control activities through participation in assigned QA programs and/or committee involvement.
  • Other duties may be assigned.
