Requirements

• Graduate of an accredited or state-approved school of practical nursing.
• Licensure as an LPN in the State of Pennsylvania.
• A minimum of one year of experience in an acute care setting.
• Phlebotomy and wound care experience required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.

Nice To Haves

• Home care experience preferred.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Perform client assessment, care planning, and intervention.
• Oversee patient care and outcomes.
• Follow physician orders and comply with the state's Nurse Practice Act, licensure/certification requirements, and company policies and procedures.
• Administer medications after passing a medication exam.
