About The Position

This position at Nuvance Health Med Practice CT assists the clinician in providing medical care to patients. The role involves preparing patients and materials for medical exams, obtaining patient history, providing direct patient care under supervision, collecting and processing specimens, administering medications and vaccinations, educating patients and families on care, and following up with patients regarding test results or clinician instructions. The position also requires adherence to compliance responsibilities, organizational values, and dependable attendance.

Requirements

Active LPN License based on state
BLS certification required within 30 days
HS Graduate or Equivalent
Graduate of an accredited LPN Program

Responsibilities

Prepares materials, supplies, equipment, and the patient to assist clinician with the patient's medical exam.
Obtains the patient's history and medical information.
Provides direct patient care as directed by a Registered Nurse or clinician.
Collects and processes specimens as directed by the clinician and performs point of care testing.
Administers medications, and vaccinations as directed by the clinician.
Instructs the patient and family on how to perform clinician recommended preventive care, takes prescribed medication, prepares for tests, monitors treatment progress and notes symptom changes.
Contacts the patient to provide requested follow-up, advises them of test results or relays clinicians instructions.
Fulfills and adheres to all compliance responsibilities related to the position.

Demonstrates regular, dependable, and predictable attendance.
Performs other duties as required.
