About The Position
Make a difference where patients feel most comfortable—at home. ChristianaCare HomeHealth is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to an adult and geriatric patient population throughout the Dover, Frederika, and Townsend, DE communities.
Requirements
- Current Delaware LPN license or Compact State license
- Minimum 1 year of LPN experience
Nice To Haves
- Home Health and/or Med-Surg experience preferred
Responsibilities
- Provide skilled nursing care and prescribed treatments in the home setting
- Care for adult and geriatric patients with a variety of healthcare needs
- Develop and implement patient-centered care plans
- Collaborate with physicians, caregivers, and interdisciplinary teams
