The FGP LPN will assist the RN in patient and family care by completing assigned patient care tasks within the scope of practice for a NYS LPN and according to NYULH policy. This role involves providing competent, safe, and compassionate care, with an understanding of the challenges associated with health maintenance, illness, hospitalization, procedures, or diagnostic testing. Responsibilities include checking vital signs, recording medical history, and utilizing specialized skills for patient care tasks such as phlebotomy, point-of-care testing (finger stick with certification), and electrocardiograms.

Requirements

• Graduate of LPN program with current NYS OP LPN licensure and registration.
• Ability to complete multiple tasks efficiently and thrive in a team work environment which pursues a positive patient care experience.
• Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities needed.
• Qualified candidates must be able to effectively communicate with all levels of the organization.

Responsibilities

• Participates in patient-centered quality and performance improvement programs consistent with service and departmental programs.
• Acquires and maintains current LPN knowledge and competence to fulfill responsibilities.
• Supports the missions, philosophy, standards, goals, and objectives of NYULH.
• Collects patient healthcare data systematically and on an ongoing basis, using appropriate assessment techniques and instruments.
• Enforces adherence to NYULH and FGP policies, procedures, and standards, including but not limited to HIPAA, Compliance, and Human Resources.
• Participates in the coordination of unit activities to provide patient care in a healing, humane, and caring environment.
• Engages in self-assessment and participates in activities to promote own professional growth and development.
• Obtains patient vital signs, weights, and EKGs as needed. Performs blood collection/phlebotomy as needed.
• Administers medications, including injections, in accordance with NYUMC clinical guidelines and NYS Scope of Practice.
• Assists with maintaining patient flow to and from exam rooms and treatment areas. Assists in escorting patients. Ensures charts are available with relevant lab work and test reports if indicated for physician review.
• Maintains an awareness of current patient-centered evidence-based practices and uses knowledge of applicable research data in patient care.
• Collaborates with members of the interdisciplinary team and promotes collegial relationships.
• Assists patients and communicates with licensed providers to obtain prescription renewals as needed.
• Performs patient-related functions as directed by the Licensed Provider and/or Registered Nurse (RN), and documents properly. These may include medication administration, vaccination, other injections, patch placement, and setup or assistance for procedures, as governed by LPN scope of practice.
• Communicates directly with patients regarding their care or treatment using appropriate terminology and a respectful manner. Interacts efficiently with medical staff and professionally with support staff.
• Serves as a support and resource for nursing staff and providers.
• Demonstrates knowledge of the NYUMC Service Standards and incorporates them into the performance of responsibilities.
• Functions as a respectful member of the health care team.
• Ensures clinical compliance with Patient Safety initiatives and reporting.
• Ensures clinical in-baskets are maintained and encounters are closed within a timely manner.
• Considers the needs and behaviors of specific patient age and cultural groups in all patient care activities.
• Completes annual competencies such as Fire Safety, Hazardous Waste, Medsled, HIPAA compliance, etc.
• Serves as an NYU Langone Health Faculty Group Practice Brand Ambassador by upholding the NYULH Mission, vision, and values and promoting excellence in the patient experience during every encounter.

• Greets patients warmly and professionally, stating name and role, and clearly communicates each step of the care/interaction as appropriate.
• Works collaboratively with colleagues and site management to ensure a positive experience and timely resolution for all patient interactions and inquiries, whether in person, by phone, or via electronic messaging.
• Proactively anticipates patient needs and participates in service recovery by applying the LEARN model (Listen, Empathize, Apologize, Resolve, Notify), and escalates to leadership as appropriate.
• Shares ideas or observed areas of opportunity to improve patient experience and patient access with appropriate leadership (e.g., ways to optimize provider schedules, minimize delays, increase employee engagement).
• Partners with Patient Access Center and Central Billing Office team members to support collaboration and promote a positive patient experience.
• Takes a proactive approach in ensuring that practice staff are fully versed in the Access Agreement gold standard principles.
