About The Position

Mercyhealth is seeking a Licensed Practical Nurse (LPN) to join their Family Practice Clinic team. This role involves direct patient care, including rooming patients, collecting assessment data, administering medications and injections, assisting with examinations and procedures, and providing patient education. The LPN will also be responsible for maintaining patient charts, coordinating referrals, performing basic lab tests and EKGs, and ensuring compliance with clinic policies and procedures. Requirements

  • Graduate of an accredited LPN Program OR Wisconsin: Have completed the portion of the RN Program needed to obtain a certificate of completion in practical nursing. Illinois: Have completed the designated curriculum of a Ladder Option Approved Designated RN Program.
  • Experience in clinical setting preferred.
  • Licensed as an LPN in the State of Practice.
  • Approved CPR or BLS certification within 90 days.

Responsibilities

  • Room patients, collect assessment data and prepare patient according to specific need.
  • Collect patient data and document in the medical record.
  • Prepare and maintain patient charts.
  • Assist in examination, treatment and other clinic procedures.
  • Administer injections and medications as ordered and directed.
  • Review physician's orders, lab requests, or follow-up needs with patient.
  • Assist with laboratory procedures, screening tests and QI projects.
  • Answer telephone and either respond to inquiry or direct caller to appropriate personnel for triage.
  • Document according to standards of care.
  • Provide patient treatments as ordered.
  • Coordinate referrals for patients through insurance and other physician offices.
  • Observe patients and report adverse reactions to medication or treatment to the provider or RN.
  • Perform the duties of a clinic LPN within alignment to LPN scope of practice.
  • Assist with clinic projects and assignments and nursing initiatives.
  • May provide feedback for evaluation or participate in peer review.
  • Assist with preparation of staff for surveys.
  • Monitor compliance with clinic initiatives as assigned.
  • Administer specified medications and nursing interventions as allowed by policy and procedure, licensure, and State Practice Act.
  • Reinforce patient teaching provided by Provider and RN.
  • Provide basic healthcare instruction to family and patient.
  • Perform EKG's and simple lab tests as needed.
  • Perform venipuncture to collect blood specimens.
  • Participate in QA & I.
  • Order and maintain supplies.
