About The Position

This position performs selected and delegated nursing tasks in the care of patients within the Diabetes Clinic. The LPN will complete patient physical assessments, perform and/or delegate patient treatments as ordered by a Provider, and participate in the education plan for patients and their families. Requirements

  • Completion of an accredited practical nursing program.
  • Current Massachusetts licensure required.
  • Basic Life Support (BLS) certification.
  • Knowledge of basic nursing procedures.
  • Strong interpersonal skills are required to gather information from individuals, build relationships that support collaborative work within the department and across the organization and provide exceptional service to patients.

Responsibilities

  • Assesses and obtains a patient's basic health status, records, and related data according to standard procedure and needs of patient and their families.
  • Administers medications, performs treatment as ordered, and operates equipment according to policy and procedure.
  • Performs venipuncture using aseptic technique, 12 lead EKGs, and administers vaccinations.
  • Participates in implementation and evaluation of care plan including long and short-term goals, nursing actions, and teaching.
  • Documents patient care according to standard procedure. Records and reports observed symptoms, reactions, treatments, and changes in patient condition to Provider and/or RN and assists in analyzing and interpreting patient data.
  • Assists provider with treatments, procedures, and special tests as required or directed.
  • Communicates with patients via electronic, telephonic, and in person, per scope of practice.
  • Collaborates with other health care providers to ensure quality and continuity of care.
  • Maintains a safe environment.
  • Complies with established departmental policies, procedures.
  • Attends a variety of meetings, conferences, seminars as required or directed.
  • Participates in the use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
