Requirements

  • Current licensure or as a Licensed Practical Nurse with the Ohio Board of Nursing.
  • One year of experience in home care, geriatrics, home health care, behavioral health, long term care, or other substitute as approved by the Ohio Department of Aging.
  • Knowledge of major chronic illnesses, disease processes, normal aging, medical terminology.
  • Excellent verbal and written interviewing and assessment skills.
  • Knowledge of local resources for older adults.
  • Excellent interpersonal and communication skills to represent our agency in a professional manner and to maintain good working relationships within our agency as well as all external entities and stakeholders.
  • Intermediate computer skills including Microsoft Office applications.
  • Ability to complete job duties by telecommunication from home and field.
  • Ability to document while interviewing in an accurate, timely manner and meet each programmatic time frames.
  • Ability to use and will be trained in the Ohio Department of Aging and Ohio Department of Medicaid documentation systems.
  • Ability to use and transport a laptop computer and case, therefore must be able to lift/carry a minimum of twenty (20) pounds.
  • Ability to ambulate in order to access private homes/locations which may not be readily accessible.
  • Must have a car available during working hours, as well as the ability to legally drive a car.

Responsibilities

  • Complete all types of assessments in the community with Consumer (and caregiver/POA/Guardian) to identify unmet needs, determine level of care, and identify any potential health and safety concerns under the supervision of an RN.
  • Educate consumers, caregivers, and stakeholders regarding eligibility of WRAAA administered programs.
  • Communicate and collaborate with caregivers, healthcare and service providers, and any other formal and informal systems to accurately determine and meet the individual’s needs...
  • Provide option s and referrals to meet individual’s needs if not pursuing Medicaid Waiver.
  • Document in appropriate software system according to contractual requirements, policies and quality compliance standards.
  • Work cooperatively and constructively with internal WRAAA staff, and with formal and informal systems.
  • Advocate on client’s behalf particularly if there is a health and safety issue or if client is at risk for institutionalization.
  • Collaborate and participate in department and agency initiatives for the purpose of improving quality, efficiency, and positive stakeholder interactions.
  • Complete assessments during core business hours and occasionally early evening or weekend hours as needed to meet the needs of the community.
  • Adhere to minimum capacity requirements. It is the sole responsibility of the CRC to meet productivity standards.
  • Participate in training, meetings, best practice committees and LEAN projects.
  • May be asked to provide assistance and back-up to other units within the department and/or other duties as assigned.
