About The Position

The Wraparound Community Resource Specialist supports the Wraparound agency by identifying, tracking, and maintaining an updated inventory of community resources and services.   This role ensures that Wraparound Facilitators and other agency staff have timely access to information connections that strengthen the support network for the youth and families.  The Wraparound Community Resource Specialist reports to the Program Director.

Requirements

  • High School Diploma or equivalent.
  • Maintain a valid Louisiana driver’s license
  • Strong understanding of The Wraparound Model
  • Good decision-making skills.
  • High standards of ethical practice, integrity, and confidentiality
  • Proficient verbal and written communication & professionalism
  • Family and youth engagement
  • Partnerships with stakeholders and community members.
  • Knowledge and experience in community-based services and support in the service area.
  • Experience working with youth and families in a community-based setting.
  • Comfortable with computers and experience in Microsoft Office programs.
  • Possess an understanding of wraparound principles and implementation.

Responsibilities

  • Participate in community-based activities that increase the community’s awareness of the wraparound program.
  • Maintain an up-to-date community resources directory and database.
  • Monitor and follow up on referrals to ensure successful linkage to the appropriate services.
  • Collaborate with internal staff to address service provider needs and service gaps.
  • Create and maintain a referral database.
  • Document all contacts, referrals, and services in accordance with agency policies and procedures.
  • Identify, develop, and maintain partnerships with community agencies, schools, healthcare providers, housing agencies, employment agencies, faith-based organizations, and other local resources.
  • Attend Families in Need of Services (FINS) court dates to obtain new referrals.
  • Facilitate provider presentations/visits to monthly staff meetings.
  • Act as a liaison between the community and the wraparound agency.
  • Provide provider updates to staff during monthly staff meetings.
  • Collaborate with the Wraparound Program Director to establish and maintain a quarterly provider meeting.
  • Maintain confidentiality as it pertains to all aspects of clients’ care, staff, and VOASCLA.
  • Participate in training and development activities as required.
  • Perform other duties as assigned.
