Phoenix Home Care and Hospice is hiring an LPN Clinical Manager to lead and support our Home Health team in Independence. This role combines clinical leadership, field support, and care coordination in a fast paced home health environment where your experience and decision making truly matter. Requirements

• Active Missouri or Compact LPN license
• Home health experience preferred
• Strong organization and communication skills
• Ability to multitask and lead in a fast moving environment
• Positive, team focused attitude

Responsibilities

• Support and guide field nurses and caregivers
• Assist with care plans and clinical oversight
• Conduct supervisory and client visits
• Help ensure quality patient care and compliance
• Communicate with physicians, families, and the care team
• Support scheduling, training, and clinical problem solving
• Be a leader your team can rely on daily
