About The Position

This full-time position involves coordinating nursing services to ensure high-quality resident care in a long-term care setting. The role includes managing various aspects of resident care such as wound care, medication pass, audits, and ADL care actions. The LPN Clinical Coordinator will also participate in care planning meetings, make recommendations for resident care, and support occupational health functions by administering employee vaccines. The position requires ensuring documentation compliance with regulatory requirements, assisting with QA activities, and overseeing the restorative nursing program. The coordinator will also be involved in the onboarding, orientation, staffing, and supervision of the nursing staff, working closely with the Director of Nursing and Nurse Manager.

Requirements

  • Licensed Practical Nurse (Required)
  • Nurse experience in long term care (Required)
  • Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)

Responsibilities

  • Coordinates nursing services, ensuring the delivery of high-quality care.
  • Assists with the coordination of care for residents in a long-term care setting, including wound care, medication pass, audits, and ADL care actions.
  • Participates in "at risk" and care planning meetings.
  • Makes appropriate recommendations regarding resident care.
  • Supports occupational health by administering vaccines to employees.
  • Coordinates with occupational health as needed.
  • Reviews reports for trends or issues and works closely with occupational health and nursing administration to resolve issues.
  • Assists with direct patient care as necessary.
  • Allows resident choices within limits of capability considering safety/dignity issues and follows policies and procedures of facility to include resident privacy and dignity.
  • Utilizes Interact process to assure proper documentation and follow through.
  • Participates in resident care plan teams.
  • Makes rounds when coming on and consults with nursing management regarding resident issues or concerns.
  • Focuses on prevention, intervention, and evaluation of wound management (pressure sores, etc.).
  • Oversees restorative nursing program and ensures therapy supplies are available as requested by Rehab department.
  • Works with DON/Nurse Manager on boarding, orientation, staffing and supervision of the nursing staff.
