About The Position
Licensed Practical Nurse (LPN) Clinical Coordinator Schedule: Monday–Friday | 8:00 AM – 5:00 PM | In-Office Position Make a Difference Where It Matters Most At Elara Caring, we believe the best place for care is where patients feel most comfortable—at home. We are seeking a dedicated Licensed Practical Nurse (LPN) Clinical Coordinator to join our growing team. In this role, you'll play a vital part in coordinating patient care, supporting clinical operations, and ensuring patients receive the right care at the right time—all while making a meaningful impact in the lives of those we serve.
Requirements
- Current LPN/LVN license
- Strong organizational and time-management skills
- Excellent communication and interpersonal abilities
- Positive, patient-centered approach to care
- Commitment to delivering exceptional clinical outcomes and service
Nice To Haves
- Minimum of 2 years of LPN/LVN experience preferred
- Home health experience preferred
- Knowledge of Medicare and Medicaid regulations, benefits, and home health practices preferred
