About The Position

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team as a Charge Nurse on the overnight shift on Freedom Bay, our Memory Care Village. In this role, you’ll provide clinical leadership and support to our nursing team while helping ensure our residents receive safe, compassionate, and person-centered care throughout the night. The Licensed Practical Nurse (LPN) is responsible for overseeing the clinical care and daily nursing operations of the assigned care area during the overnight shift. Working on our Memory Care Village, this position plays an important role in supporting residents with dementia and other cognitive needs while providing leadership, guidance, and support to CNAs and other nursing staff. The Licensed Practical Nurse (LPN) serves as a clinical resource, responds to changes in resident condition, coordinates care, and promotes a safe, respectful, and consistent environment for residents and staff. This position works independently during the overnight hours while collaborating with the interdisciplinary team to ensure continuity of care.

Requirements

Current, active LPN license in the State of Maine
Graduate of an accredited nursing program
Ability to work harmoniously and professionally with co-workers, residents, families, and supervisors
Ability to exercise discretion and maintain confidentiality
Ability to work effectively both independently and as part of a team
Proficiency in spoken and written English
Basic computer skills

Nice To Haves

Experience working with individuals with dementia or in a memory care setting is preferred

Responsibilities

Provide direct nursing care to residents in accordance with care plans, policies, and best practices
Provide compassionate, person-centered care to residents living with dementia and other cognitive impairments
Supervise, support, and provide direction to CNAs and other nursing staff on the overnight shift
Coordinate and delegate assignments to ensure appropriate staffing and workload distribution
Monitor resident conditions and respond promptly and appropriately to changes in status
Administer medications and treatments within the scope of licensure and organizational policy
Communicate effectively with providers, families, and members of the interdisciplinary team
Ensure accurate and timely documentation of care and services provided
Promote a calm, safe, and supportive environment for residents throughout the overnight hours
Follow appropriate approaches for residents experiencing confusion, anxiety, behavioral changes, or other dementia-related needs
Assist with admissions, discharges, and care transitions as needed
Address concerns, incidents, and emergencies in a timely and professional manner
Support compliance with state and federal regulations and organizational policies
Serve as a clinical resource and role model for staff, fostering a positive and collaborative team environment
