About The Position

The LPN Charge Nurse at The Oaks of Clearwater is responsible for planning, supervising, and evaluating all personnel assigned to the nursing unit to meet the total needs of the patients. This role ensures the quality of nursing service in accordance with federal, state, and local regulations, as well as established policies and procedures, to maximize the fulfillment of care-giving needs for patients residing at The Oaks of Clearwater. The position requires making prompt and accurate nursing care and supervisory judgments, administering nursing bedside care specific to the elderly and others needing long-term care services, and effectively supervising assigned units with regard to residents, employees, families, visitors, and the public. Strong communication skills are essential for interacting with residents, staff, families, visitors, and others, conveying a sense of caring, concern, and dignity for the residents.

Requirements

  • Current, unencumbered license to practice as a nurse in the State.
  • At least six months experience working in a hospital, long-term care facility or other healthcare institution with training or experience implementing total patient care and care planning and in making sound nursing judgments.
  • Familiarity with the policies, regulations and procedures of assisted nursing homes.
  • Capable of lifting a minimum of forty pounds and may need to assist in lifting 100+ pounds.
  • Ability to work in the environment conducive to caring for residents without posing a direct threat to self or others.
  • Neat, clean odor-free grooming and dress as specified in the personnel handbook.
  • Report promptly to the assigned area at the start of each shift and become informed on the current condition of all assigned residents, ready for routine duties and emergencies.
  • May include working on-call duty.

Nice To Haves

  • Taken courses in nursing management or supervision.
  • Taken courses in geriatric rehabilitative and restorative nursing.

Responsibilities

  • Give and receive condition report from the on-coming and off-going shift.
  • Give direct nursing care to residents.
  • Direct the day-to-day functions of Nursing assistants and other staff being supervised.
  • Make rounds and routine checks on patients.
  • Maintain inventory of patient drugs and medications on the unit, unless medication nurse is on duty.
  • Prepare required forms for admitting, discharging and transferring patients.
  • Notify the physician when resident’s condition warrants.
  • Perform administrative duties such as the documenting of Nurse’s notes, Doctors’ orders, Discharge plans, Dietary plans, Reports, Patient Assessment and Comprehensive Care Plan, Billing and other charge slips, Miscellaneous forms, charts, etc.
  • Chart pertinent facts and information in a professional, clear and comprehensive manner.
  • Admit, transfer and discharge residents according to policy.
  • Recognize and respond to changes in resident’s conditions and document observations, interventions and outcomes.
  • Monitor and/or record follow through for signatures of telephone orders from physicians.
  • Conduct and/or monitor prescribed medications.
  • Oversees LPN performance, assists them and provides education as needed.
  • Perform emergency procedures such as Cardiopulmonary Resuscitation and, in the event of death, notify family and funeral home.
  • Recognize, intervene and report accidents and incidents, when they occur.
  • Monitor and update care plans, each shift, as appropriate.
  • Supervise and participate in fire safety and other disaster drills, and be prepared to carryout duties, in the event of an actual fire or disaster.
  • Recognize, intervene and/or report complaints and grievances from residents, families, visitors and employees.
  • Monitor the unit for possible health and safety hazards.
  • Maintain privacy and confidentiality of records, conditions and other information relating to residents, employees and facility.
  • Follow the established universal precautions and isolation procedures.
  • Notify supervisor of pending absence or tardiness within the time frame described in the personnel handbook.
  • Encourage an atmosphere of optimism, warmth and interest in residents’ personal and healthcare needs.
  • Follow established smoking regulations and report violations.
  • Perform other related duties, as assigned by supervisor.
  • Update daily census reports and submit as required.
  • Attend and participate in orientation programs, on-going training and educational classes.
  • Inventory and request equipment, supplies, etc.
  • Order from pharmacy, prepare and administer medications as ordered by Physician and assigned.
  • Assist in updates of written policies and procedures of routine nursing care.
  • Participate in surveys made by authorized government agencies.
  • Complete remainder of medical forms, reports, evaluations, studies, charting, etc., not considered as essential functions.
  • Monitor and participate in daily clean-up of the nurse’s station.
  • Wound care and documentation on stages.
