About The Position

The LPN Charge Nurse supervises, provides, and directs nursing care in compliance with company and facility policies and procedures. This role involves making advanced observations, reporting and recording them appropriately, forming nursing judgments, and carrying out nursing actions based on scientific principles. The position operates under the general supervision of the Director of Nursing.

Requirements

  • Current Nurse License
  • Graduate of an accredited school of nursing or a vocational school of practical nursing
  • Working knowledge of Windows, Microsoft Office Suite and the Internet
  • Good written and verbal communication skills
  • Good communication, comprehension and interpersonal skills
  • Ability to speak, read, write and understand English

  • One to two years of experience preferred

Responsibilities

  • Supervise, provide, and direct nursing care in compliance with facility policies and procedures.
  • Make advanced observations, report and record them appropriately, form nursing judgments, and carry out nursing actions.
  • Follow established standards of nursing practices and assume responsibility for compliance with federal, state, and local regulations.
  • Interpret existing policies to staff, families, and physicians.
  • Perform daily rounds and develop/implement comprehensive care plans based on resident needs.
  • Follow policies and procedures for passing medications and treatments, and manage medication inventory.
  • Follow staffing guidelines and make nursing assignments.
  • Receive and give accurate reports on resident conditions for continuity of care.
  • Monitor diets and supervise meal trays and snacks, assisting as needed.
  • Follow facility admissions procedures and communicate admissions to relevant parties.
  • Ensure employees follow the chain of command and personnel policies.
  • Assign nursing assistants, ensure efficient completion of assignments, and supervise/correct performance issues.
  • Participate in performance evaluations, counseling, coaching, and training of nursing assistants.
  • Communicate professionally and effectively, following written and verbal directions.
  • Provide exemplary customer service to residents, peers, families, managers, and visitors.
  • Report changes in physical or mental conditions on a timely basis.
  • Follow all company, facility, and state policies and procedures.
  • Attend staff meetings, in-services, and other assigned meetings.
  • Exercise keen observations of environmental conditions and cooperate with staff to ensure quality conditions.
  • Document resident information timely, accurately, and confidentially.
  • Document changes and notifications to POA and doctor, and perform comprehensive assessments.
  • Verify MARs/TARS are correctly updated.
  • Adhere to Resident Rights documentation guidelines, ensuring care is provided with respect and dignity.
  • Know resident rights and help residents exercise/protect them.
  • Report resident complaints and maintain confidentiality.
  • Adhere to all policies and procedures under Safety & Compliance.
  • Ensure residents are safe, including proper use of Wanderguard systems.
  • Demonstrate proper use of equipment, report needs or repairs.
  • Use protective equipment and follow infection control protocol standards.
  • Practice universal precautions and life safety measures.
  • Demonstrate and maintain a positive, professional manner, manage time efficiently, and provide leadership.
  • Collaborate and participate in policy creation and problem-solving.
  • Adhere to all policies, procedures, terms, and conditions set forth in the Employee Information Guide, Code of Conduct, and facility handbook.
  • Complete all annual education requirements timely.
  • Perform other duties as assigned.
