About The Position

Annandale Village is a nonprofit community in Suwanee, GA, serving adults with developmental disabilities and acquired brain injuries. We offer a full continuum of care on our 55-acre campus, including assisted living, personal care homes, a skilled nursing facility, a day program, and an independent living program. Requirements

  • Graduate of an accredited school for Licensed Practical Nursing and currently licensed as an LPN in Georgia.
  • Three to five years of recent LPN experience in GA.
  • Supervisory experience required.
  • Must be able to supervise and instruct CNAs and others, and to exercise independent judgment.
  • Ability to respond to emergencies; ability to respond to and seek supervision.
  • Able to read and write in English, to understand and follow written and oral instructions, and able to communicate with residents, personnel, vendors, and consultants.
  • Must be at least 18 years of age.
  • Must have a valid Class C Georgia Driver’s License.
  • Must have proof of vehicle insurance.
  • Must meet company policy regarding MVR requirements.
  • Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
  • Must undergo and pass a drug screening and screening for Tuberculosis.

Responsibilities

  • Supervises the day-to-day nursing activities of the facility in accordance with current federal, state, and local standards.
  • Exercises supervisory authority and independent judgment to ensure proper discipline and productivity.
  • Directs the day-to-day functions of nursing activities to include supervising CNAs to ensure residents needs are met.
  • Manages CNA duties, documentation completion, and initiates and oversees daily shift huddle meetings.
  • Educates and trains new staff and introduces new procedures and protocols, including orientation.
  • Communicates with management regarding CNA performance, staff issues, and changes to treatment and protocols, providing and documenting teachable moments.
  • Works with the Scheduler to ensure regulatory coverage is met, managing overnight and weekends call outs.
  • Evaluates resident complaints to determine the appropriate plan of care, communicates with physicians and other healthcare providers as necessary.
  • Carries out physician’s orders regarding diet, medication, and treatment.
  • Performs assessments; changes dressings; monitors and ensures quality care through observation and evaluation.
  • Reports on physiological and mental health status of residents.
  • Documents nursing interventions on medical records.
  • Distributes and maintains records of controlled drugs and ensures their security.
  • Makes decisions in routine cases; instructs residents, staff, and family members regarding treatments, nursing plans, and related patient care issues.
  • Remains calm under pressure, competent in making quick decisions in emergency situations, evaluating the quality of resident care, resolving conflicts, and anticipating problems before they arise.
  • Maintains availability of medications and supplies.
  • Works with residents with developmental disability, intellectual disability, or an acquired brain injury.
  • Maintains privacy and confidentiality; treats residents with kindness, dignity, and respect; and ensures that all nursing personnel know and comply with the Resident’s Rights rules.
  • Communicates medical information, change in resident health status, and health-related concerns to the resident’s family/guardian.
  • Performs and/or supervises the medication administration processes, administration of prescribed treatments, and other therapies, assuming this responsibility for both units during occasional staffing shortages.
  • Monitors, evaluates, and documents residents' prescribed blood sugar levels, vital signs, and weights, alerting the physician of any findings outside of established normal ranges.
  • Collaborates with the Dietician and the Culinary Operations Director in planning for residents requiring special diets.
  • Collaborates with therapy regarding the plan of care.
  • Receives Physician’s orders, updates MAR’s with new medication orders and change orders, and communicates same to other staff assisting with medications.
  • Performs admission, transfer, and discharge processes.
  • Facilitates the development of the resident’s preliminary and comprehensive assessment and written care plan.
  • Completes nursing documentation to ensure accuracy, timeliness, and completeness.
  • Administers professional services and provides direct hands-on-care as necessary.
  • Maintains professional competencies through education and training.
  • Attends all required and assigned trainings.
  • Performs other duties as assigned that may be necessary in the best interest of Annandale at Suwannee, Inc. to meet the patient care needs of the Residents.
  • Serves as a member of or provides input to the Professional Staff Committee, the Process Improvement Committee, and the Resident’s Council.
