About The Position

The LPN Charge Nurse supervises, provides, and directs nursing care in compliance with company and facility policies and procedures. This role involves making advanced observations, reporting and recording them appropriately, forming nursing judgments, and carrying out nursing actions based on scientific principles. The LPN Charge Nurse is responsible for daily rounds, developing and implementing care plans, passing medications and treatments, ordering and crediting medications, following staffing guidelines, monitoring diets, and assisting with admissions. They also supervise nursing assistants, provide counseling and training, and participate in performance evaluations. Effective communication with residents, families, staff, and physicians is crucial, as is timely and accurate documentation of resident information. Adherence to resident rights, safety protocols, and infection control standards is paramount.

Requirements

Current Nurse License
Working knowledge of Windows, Microsoft Office Suite and the Internet.
Good written and verbal communication skills.
Good communication, comprehension and interpersonal skills.
Ability to speak, read, write and understand English.

Nice To Haves

One to two years of experience preferred.

Responsibilities

Supervise, provide and direct nursing care in compliance with facility policies and procedures.
Make observations on an advanced level; report and record observations appropriately; form nursing judgments and carry out nursing actions.
Follow established standards of nursing practices and assume responsibility for compliance with federal, state and local regulations.
Interpret existing policies to staff, families and physicians.
Perform daily rounds, develop and implement accurate comprehensive care plans based on each resident’s needs.
Follow established policies and procedures for passing medications and treatments.
Order and credit medications as needed, keeping accurate accounting of all medications (Stock medications, unit dose and narcotics).
Follow staffing guidelines and make nursing assignments.
Receive and give an accurate report on resident’s condition to provide continuity of care.
Monitor diets and supervise meal trays and snacks served to residents and the resident’s intake.
Assist in passing meal trays, feeding and passing of snacks.
Follow facility admissions procedures and communicate admission to other departments, staff, physicians and other service providers.
Ensure employees follow the chain of command in a manner consistent with the personnel policies.
Assign nursing assistants and ensure the efficient completion of their assignments.
Supervise, document and correct poor performance of nursing assistants.
Keep Unit Managers/Director of Nursing informed on a timely basis of poor performance problems.
Participate in the performance evaluation process of nursing assistants and provide counseling, coaching and training.
Present performance evaluations to staff.
Follow written and verbal directions.
Provide exemplary customer service to residents, peers, families, managers, and visitors.
Report any changes in physical or mental conditions on a timely basis.
Follow all company, facility and state policies and procedures.
Attend staff meetings, in-services, inter-disciplinary team and committee meetings as assigned.
Exercise keen observations of noise, odors, cleanliness, etc.
Cooperate with other staff members in all departments to ensure quality environmental conditions for the residents and staff.
Document resident information timely, accurately and confidentially according to the procedures.
Document changes and notification to POA and doctor; perform comprehensive assessments as assigned.
Verify change-over MARS/TARS are correctly updated and necessary changes are made to new MAR/TAR by the 1st of each month.
Know resident rights and help the residents exercise and/or protect their rights.
Report resident complaints to supervisor in charge, maintain confidentiality.
Ensure that residents are safe (for example: Wanderguard tab is securely attached to bed/chair as ordered, Wanderguard bracelets are working properly).
Demonstrate proper use of equipment.
Report equipment needs or repairs.
Use protective equipment, follow infection control protocol standards, policies and procedures.
Practice universal precautions and life safety measures.
Manage time efficiently and effectively.
Provide leadership as a role model to promote a team approach.
Collaborate and participate in policy creation and appropriate problem solving methods.
Complete all annual education requirements timely, including but not limited to Compliance, Privacy, Security and HIPAA training and exhibit behavior as set forth by the Code of Conduct in the performance of their duties.
