About The Position
We are looking for an LPN to join our Family Medicine Team as a Care Coordinator! In this role, you will have the opportunity to help support patients by collaborating with patients to set healthcare goals, complete advance care plans, and track providers' quality goals.
Requirements
- Completion of post secondary program in Practical Nursing
- Nursing degree
- Current Ohio LPN license
- Knowledge of Microsoft Office
Nice To Haves
- Experience in physician offices
- BLS/CPR certified
- Proficient with technology required for data tracking and reporting
