About The Position

The LPN Care Coordinator position serves as a liaison, promoting coordination, communication and collaboration among Brookdale teams, residents, families and healthcare providers within the community. This position works closely with primary and specialty care providers establishing care-plans. The LPN Care Coordinator will also serve as the resident advocate. Previous case management experience is required. This position will be responsible for overseeing Brookdale communities.
