About The Position

The LPN Care Coordinator position serves as a liaison, promoting coordination, communication and collaboration among Brookdale teams, residents, families and healthcare providers within the community. This position works closely with primary and specialty care providers establishing care-plans. The LPN Care Coordinator will also serve as the resident advocate. This position will be responsible for overseeing 3 Brookdale communities.

Requirements

  • LPN (Licensed Practical Nurse)
  • Experience in care coordination, communication, and collaboration.
  • Ability to work with primary and specialty care providers.
  • Ability to serve as a resident advocate.
  • Veterans, transitioning active duty military personnel, and military spouses are encouraged to apply.
  • Qualified associates who have been with Brookdale for at least a year may be eligible for advancement fees for naturalization (Form N-400) application costs, up to $725, less applicable taxes and withholding.
