About the Role As a Licensed Practical Nurse (LPN) in Home Health, you'll provide high-quality nursing care to patients in their homes under the supervision of a Registered Nurse and in collaboration with an interdisciplinary care team. You'll play an essential role in helping patients manage their health, maintain independence, and improve their quality of life. Requirements
- Graduate of an accredited Practical Nursing program.
- Current, unrestricted Massachusetts LPN license.
- Successful completion of NCLEX-PN.
- Current CPR certification.
- Strong clinical assessment, communication, and documentation skills.
- Valid driver's license, reliable transportation, and automobile insurance.
- Ability to meet required health screening and clearance requirements.
- Professional references demonstrating clinical competence and reliability.
Nice To Haves
- Associate degree.
- Two or more years of nursing experience.
- Experience in home health, hospice, long-term care, community health, or related settings.
Responsibilities
- Provide skilled nursing care according to individualized care plans.
- Administer medications and treatments safely and accurately.
- Monitor patient conditions and report changes to the supervising RN.
- Collaborate with physicians, case managers, caregivers, and healthcare team members.
- Educate patients and families on disease management, medications, and treatment plans.
- Maintain accurate and timely clinical documentation.
- Support patients with dignity, compassion, and respect.
- Participate in care conferences, team meetings, and ongoing training.
