This role involves conducting initial assessments and reassessments for members/clients within the elderly and disabled waiver program. The primary goal is to ensure members can remain in their community by assessing their ability to complete Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), and coordinating necessary services. The position requires diligent documentation, communication with team members and supervisors, and adherence to all company policies, state, and federal regulations.
