About The Position

This role involves conducting initial assessments and reassessments for members/clients within the elderly and disabled waiver program. The LPN Assessment Nurse will visit clients in their homes to assess their health status, determine their ability to complete Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), and ensure they have the necessary services to remain in the community. The position requires adherence to agency policies, state and federal regulations, and participation in team meetings and in-service training.

Requirements

Current and unrestricted Licensed Practical Nurse (LPN) licensure in state of practice.
Minimum of two (2) years practical nursing experience.
Experience working with elders and disabled populations.
Computer literate and ability to learn new programs.
Ability to complete tasks in compliance with timelines created by the Department of Community Health (DCH).
Ability to communicate with translators for non-English-speaking clients/members.
Keeps supervisor informed of daily activities and issues.
Demonstrates ability to get along with co-workers; maintain effective relationships with management staff and the general public.

Follows all company policies and procedures, state and federal laws and regulations and accreditation requirements.
Reports any violations to the appropriate supervisor and/or compliance department.

Responsible for assisting with implementation of the waiver program guidelines and client/member goals working with the Medical Director and Home First team.

Able to respond to change productively and handle additional projects as assigned, demonstrates flexibility daily to meet needs of the agency patient/members.

Nice To Haves

Three (3) years practical nursing experience.
Previous governmental waiver program experience preferred.
Any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.

Responsibilities

Completes initial and annual assessments in client/member’s home and determines member ability to complete IADLs and ADLs.
Completes modified assessments as needed for transfers, hospital and nursing home discharges, and increased or added services.
Assures members have services to remain in their community and fulfills the fiduciary obligation to meet members’ needs based on guidelines.
Communicates monthly with administrator to receive re-assessment schedule for SOURCE and CCSP annual review.
Reviews medications for appropriateness, noting any new, changed, or discontinued medications.
Documents assessment entries into Harmony system within established guidelines.
Requests medical and hospital records as needed to assist with determining eligibility.
Participates in weekly team meetings with the Medical Director to present new admits, reassessments, hospitalizations, Home Health admits and discharges, repeat variances, potential discharges, actual discharges, and situations threatening health or functional status.
Attends and participates in mandatory in-services.
Complies with corporate compliance program.
Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
Follows established safety regulations, including fire protection & prevention, smoking regulations, and infection control and prevention.
Follows established safety procedures when performing tasks and/or working with equipment.
Visits clients/members in their residence to provide care or services according to rules and regulations for elderly and disabled waiver program guidelines.
Assesses the health status of assigned clients/members according to agency policies and procedures and scope of practice guidelines.
Meets agency productivity requirements and utilizes time and resources effectively.
Performs other duties as assigned by Administrator.
Participates in the agency in-service education program.
