About The Position

Easterseals PORT Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Virginia REACH team. This role supports individuals with intellectual/developmental disabilities and co-occurring behavioral health needs within our Adult Crisis Therapeutic Home (YCTH) in Woodbridge, VA. The LPN provides medical support, administers medications, conducts nursing assessments, offers health education, and ensures regulatory compliance. This position involves collaboration with interdisciplinary teams, maintaining accurate documentation, and supporting the overall health and wellness of individuals. The role offers a consistent schedule and a modern therapeutic home environment.

Requirements

Current valid, unrestricted license as a Practical Nurse in the state of Virginia.
Ability to provide care in a crisis group home.
Passion for helping others.
Experience with MS Office suite and data entry.
Strong verbal and written communication skills.
Valid driver's license and car insurance.

Nice To Haves

Knowledge of the MH and I/DD systems of care.
Experience with crisis prevention and intervention services.
