This is an exciting opportunity to become part of the Primary Care Network team! This Family Medicine practice provides outpatient services for all ages from Newborn to Senior, including preventative health maintenance, and chronic and acute care needs. The LPN will be part of a team comprised of Ambulatory Techs, RNs, Physicians and APP’s. Responsibilities include direct patient care, administration of medication/vaccines, minor laboratory testing, and communication with patients and providers using EMR. Looking for a candidate that is a self-starter, team oriented and enjoys collaborating with others.

Requirements

• At least 6 months in direct nursing care preferred but not required.

Nice To Haves

• Pediatric experience preferred.

Responsibilities

• Provision of direct care to assigned patients in an outpatient setting.
• Contributes to unit activities and supports enhancement of the care delivery system.
• Works as a team member and assists other staff members with implementing patient care.
• Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD.
• Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication.
• Direct patient care.
• Administration of medication/vaccines.
• Minor laboratory testing.
• Communication with patients and providers using EMR.
