Requirements
- Board certification or board-eligible internal medicine, geriatrics, critical care, or family practice
Nice To Haves
- Previous experience as a hospitalist or in leadership positions is strongly preferred.
Responsibilities
- Lead care teams in skilled nursing facilities
- Manage complex cases discharged early from hospitals
- Collaborate with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting
- Ensure continuity, quality, and compassion in every patient interaction
