About The Position

The Long-Term Care Registered Nurse (RN) delivers comprehensive nursing care to residents across three long-term care units, including two 45-bed units for long-term and short-stay residents, and one 10-bed unit providing hospice and palliative care services. The RN is responsible for providing individualized, age-specific care that focuses on enhancing quality of life, promoting comfort, and supporting residents' health needs throughout their stay. This role requires strong clinical skills, compassionate care delivery, and effective communication to meet the unique needs of long-term care residents and their families in a supportive and patient-centered environment.

Requirements

  • BLS certified
  • At least three (3) years of experience in acute care or other areas.
  • Possess an unrestricted license to practice medicine in one of the States, Territories, or of the United States or the District of Columbia.

Responsibilities

  • Assesses, plans, implements, and evaluates care based on age-specific components.
  • Coordinates care focused on patient transition through the continuum of care.
  • Provides family education.
  • Responsible for patient ADLs (activities of daily living).
  • Performs assessments.
  • Administers BCMA medication and procedures per established policies and guidelines (i.e.).
  • Administers Barcode medication and administration.
  • Performs IV therapy.
  • Administers blood transfusion.
  • Monitors blood pressure, weight, and hydration.
  • Administers tube feeding.
  • Manages behavioral patients.
  • Participates in interdisciplinary team meetings.
  • Handles admissions and discharges.
  • Manages internal and external transfers.
  • Delegates tasks to ancillary staff.
  • Documents in the electronic medical record.
  • Transports patients.
  • Responds to emergency procedures.
