About The Position
We are currently seeking qualified candidates for a full time Long Term Care Liaison - LVN position in the Clear Lake area. Job Description: The Long Term Care Liaison LVN positions works closely with the Clinical team, patients, families and nursing facilities.
Requirements
- Graduate of an accredited college or university or two years of experience working in hospice and/or home health patient care
- Must be a Licensed Vocational Nurse (LVN) in the State of Texas
- Experience with long term care - required
- Must have good oral communication and writing skills
- Must be able to speak well in public
- Must be able to practice the principles of good public relations
- Must have a current state driver's license
- Must have reliable transportation
- Must have a current auto insurance policy
- Clean background check and drug screen
