About The Position
MedCap Health is seeking a full-time LongTerm and Post-Acute Care APP to help reshape the future of post-acute care. This role offers flexibility, autonomy, and a true sense of purpose for compassionate and skilled APP/NP/PA professionals.
Requirements
- A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
- 1+ years of experience in post-acute/SNF/LTC
- Prior post-acute, long-term, and/or skilled nursing facility experience
Responsibilities
- Manage patients with acute and chronic diseases.
- Collaborate directly with the attending physician to provide the highest quality of care.
- Effectively manage patients with debilitating chronic diseases, improving their quality of life.
- Communicate and work seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care.
