About The Position

MedCap Health is seeking a full-time LongTerm and Post-Acute Care APP to help reshape the future of post-acute care. This role offers flexibility, autonomy, and a true sense of purpose. As a key member of a multidisciplinary team, you will play a central role in patient care. You will spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.

Requirements

A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
3-5 years of experience as an RN or post-acute role specializing in cardiology
1+ years of post-acute, long-term, and/or skilled nursing facility experience in cardiology

Responsibilities

Managing patients with acute and chronic diseases.
