About The Position

The Transplant Coordinator delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. This job safely administers prescribed medications, identifying incompatibilities and/or interactions. This job manifests a high level of expertise in the assessment, coordination and care of the complex patients, families, or communities to actual or potential health problems; prevention of illness and injury; maintenance of wellness; and provision of comfort. The Transplant Coordinator demonstrates strong clinical experiences in patient care. The position demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex patients and their responses to actual or potential health problems. The position assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. The position also provides direction, education, and leadership.

Requirements

  • Current RN licensure in Virginia or eligible
  • Current RN licensure in Virginia
  • AHA BLS HCP Certification or equivalent
  • Minimum of three (3) years of nursing experience in transplant, critical care or medical-surgical nursing
  • Two (2) years of progressive leadership experience
  • Demonstrated clinical expertise for population served with a minimum of one (1) year in specialty area
  • Baccalaureate Degree in Nursing from an accredited School of Nursing

Nice To Haves

  • Five (5) years of clinical experience in the transplant field
  • Academic Healthcare experience
  • Advanced Practice Nursing Role or Clinical Leadership experience
  • Masters Degree in Nursing
  • Certification in Transplantation

Responsibilities

  • Delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care.
  • Safely administers prescribed medications, identifying incompatibilities and/or interactions.
  • Demonstrates strong clinical experiences in patient care.
  • Assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort.
  • Provides direction, education, and leadership.
  • Follows documented physician/licensed independent provider orders, within the boundaries of the Nurse Practice Act and established standards, policies and practice guidelines.
  • Organizes and plans work with input from the patient/family with specific outcomes and demonstrated use of sound clinical judgment and resources.
  • Handles all day-to-day operations of the role independently with minimal assistance.
  • Seeks advice and guidance from the Manager or Director on an as needed situational basis and when there is a major impact on activity outside the unit.
  • Performs daily operational functions and leadership of staff without prior approval.
