The Lifestyle Companion is a compassionate and resourceful professional dedicated to enhancing the quality of life for individuals living with dementia and their unpaid caregivers (typically family members or loved ones). This role focuses on social engagement, household organization, and lifestyle coordination, creating meaningful, joy-filled experiences that support both the individual living with dementia and their caregiver. As part of Companioa's integrated service offerings, the Lifestyle Companion works under the Companioa umbrella with the guiding philosophy that: "No one should be on this journey alone." Managing the effects of dementia can be complex and overwhelming. Our Lifestyle Companion services provide a highly personalized and elevated level of support, ensuring that each client receives care tailored to their unique needs and preferences. Interdisciplinary Team Model & Integrated Care Approach: Operating within an interdisciplinary team model, the Lifestyle Companion works closely with, and reports to, the Guide (LCSW/MSW) while contributing to the Interdisciplinary Team (IDT). This is a per diem role and will average between 10 and 15 hours per week.
