As the Lifestyle Coach, you will provide support and guidance to program participants enrolled in CentroMed’s Chronic Disease Prevention Program. You will implement the standard curriculum designed for the program, including Diabetes Prevention: Prevent T2. The Lifestyle Coach identifies habits that may be affecting a participant's health and introduces healthier habits and physical activity programs. In addition, Lifestyle Coaches routinely outreach to their existing eligible client base and the community at large, particularly those residing in unincorporated areas of the county and smaller communities outside of San Antonio. The goal will be to inform and motivate individuals into action for their quality of life, that of their family, and community. The Lifestyle Coach serves as an advocate for improving the health of participants in the Chronic Disease Prevention Program. The Lifestyle Coach communicates with participants and develops strategic plans based on participants’ needs and concerns. Furthermore, the Lifestyle Coach maintains communication with patients via text or email to motivate, reinforce physical activity, and track goals. The Lifestyle Coach will work closely with CentroMed’s Registered Dietitian and Program Director to provide a comprehensive wellness plan for participants.
