The Life Set Specialist is responsible for providing services to address the needs of young people 17.5-21 who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood. This includes obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system, and remaining free from legal involvement. The role involves establishing/re-establishing meaningful relationships with parents, siblings, or other significant adults, and coordinating family contact as appropriate. The specialist assures compliance with all service requirements of the DCFS, including service plans, sibling visitation, and court hearings, and manages all aspects of discharge planning in coordination with the team.
