As the Lifestyle coach you will provide support and guidance to program participants enrolled in CentroMed’s Chronic Disease Prevention Program and implement the standard curriculum designed for the program incorporated: Diabetes Prevention: Prevent T2. The lifestyle coach identifies habits that may be affecting the participant’s health and introduces healthier habits and physical activity programs. In addition, Lifestyle Coaches routinely outreaches not only its existing eligible client base but the community at large, particularly those residing in unincorporated areas of the county and smaller communities outside of San Antonio. At a minimum, target communities will include Adkins, Atascosa, Converse, Losoya, Macdona, Sayers, Somerset, and Wetmore. The goal will be not only to inform – but to motivate them into action – for their quality of life, that of their family, and community. The Lifestyle coach serves as an advocate for improving the health of participants in the Chronic Disease Prevention Program. The Lifestyle Coach communicates with participants and develops strategic plans based on participants’ needs and concerns. Furthermore, the Lifestyle Coach maintains communication with patients via text or email (based on the preferred mode of communication by the participant) to motivate, reinforce physical activity, and track goals. The Lifestyle Coach will work closely with CentroMed’s Registered Dietitian and Program Director in order to provide a comprehensive wellness plan for participants.
