About The Position

Kentucky River Community Care is seeking a Life Skills Coach/Transporter/Community Support Associate for their Perry County Health & Wellness location in Hazard, KY. This is a Regular Full-Time position. The role involves teaching clients essential life skills such as accessing community resources, medication adherence, interpersonal skills, behavioral support, crisis coping, emotional regulation, transportation access, financial budgeting, and independent community shopping. The position also requires completing documentation in EMR within 24 hours of service delivery, conducting activities to promote client independence, enhancing community living through skills training and supervision as per treatment plans, and maintaining all required trainings and supervision. The role may also involve participation in therapeutic rehabilitation and 1915i Rise services.

Requirements

High School diploma or GED
One year of full-time experience working with individuals who receive services for treatment of a behavioral health disorder.
Valid driver's license with 2 years of driving experience.
Conscientious

Good organizational and time management skills

Responsibilities

Teaching clients skills such as how to utilize/access community resources, medications adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, how to access transportation, how to budget/manage finances, and how to shop independently in the community.
Complete documentation in EMR within 24 hours of service delivery.
Conduct activities necessary to allow individuals to live with maximum independence in the community.
Enhance community living through utilizing skills training, cueing or supervision as identified in recipients treatment plan.
Maintain all trainings and required supervision.
Participate in therapeutic rehabilitation and 1915i Rise services as needed.
