Maggie's Place is an innovative, beautiful, 5-star, boutique-style senior living community dedicated to enriching the lives of its residents through purpose, passion, and joy. The community offers various levels of living, including independent living, assisted living, short-term rehab, and long-term care, as well as a village-style memory care community designed with the latest advancements for a safe, secure, and comforting environment. This role is for a dedicated Life Services Coordinator - CNA to join the Maggie's Place Memory Care team, responsible for cultivating an environment that maximizes each resident’s independence. The coordinator will provide assistance with personal care, medications, social and recreational activities, meals, and other tasks to ensure the continued well-being of tenants.

Requirements

  • Must have 1-2 years’ previous experience working with individuals with dementia, preferably in a memory care setting.
  • Must be at least 18 years of age, with a high school diploma or equivalent GED.
  • Must have a valid CNA in the state in which applying.

Responsibilities

  • Provides services to tenants as assigned and as indicated on Resident Service Plans, which may include grooming and dressing guidance, assistance with bathing, and medication reminders/assistance.
  • Report any significant tenant changes to the nurse.
  • Monitors for changes in tenant needs, preferences, and/or health status, and reports/documents any changes according to established procedures.
  • Reviews the Staff 24-Hour Communication log and corresponding Tenant Service Notes on a daily basis for information about changes in tenant service needs and/or health conditions.
  • Prepare and serve meals/snacks, monitor food expiration dates and maintain kitchen cleanliness.
  • Complete various housekeeping duties including: dusting, vacuuming, cleaning bathrooms and common spaces, maintaining kitchen cleanliness, make and change bed linens, and laundry services.
  • Maintain activity calendar and engage tenants in activities such as spiritual, educational, games, music, and physical activities.
  • Provide companionship.
  • Works effectively with minimal supervision and in a team setting.
  • Follows safety policies in performing care, assuring resident safety.
  • Follows and upholds policies and procedures.
  • Attends and participates in facility meetings and in-service training programs as assigned.
  • Follows infection control guidelines.
  • Observes residents’ rights including, but not limited to, privacy, dignity, etc.
