About The Position

Supports the Life Enrichment Team alongside the Director of Life Enrichment. Takes lead responsibility in planning, leading, and supporting life enrichment programming in Health, including large and small group activities, one-to-one visits, and special events. Assists with scheduling staff, coordinating volunteers, maintaining resident lists including veterans, birthdays, bus trip outings and supporting team operations. Completes Health Care documentation requirements including assessments, MDS/QR interviews, careplans. Attends interdisciplinary careplans and HC team meetings in the absence of director. Maintains monthly calendars and supports household integration under the person-centered care model.

Requirements

  • Must be at least 18 years of age.
  • High school diploma/GED or demonstrated proficiency in reading and writing skills required.
  • Proficient in use of computer and related technology required.
  • Must be proficient with email (including opening/closing attachments), Microsoft Word, Excel, and electronic medical record systems.
  • Good verbal, non-verbal, and written communication skills required.

Nice To Haves

  • Prior experience in related role preferred.
  • Prior experience in a long-term care facility preferred.

Responsibilities

  • Takes primary responsibility for planning and carrying out activities for assigned Health Care households, including monthly activity calendars.
  • Meets with new residents to assess interests and completes an Activity Assessment, 'Getting to Know You' interviews.
  • Completes required MDS paperwork within regulatory timelines including careplan documentation and progress notes as well as additional documentation in EMR as needed.
  • Leads and assists with activity programming, including therapeutic recreation, 1:1 visits, and special events.
  • Regularly engages in creative planning and collaborates with the Life Enrichment team to develop meaningful programs.
  • Participates in interdisciplinary care planning, rounds meetings, and QA meetings as needed.
  • Supports and trains new Life Enrichment team members, including orientation, tours, and scheduling.
  • Assists with team scheduling, including HR training sessions, PRN coverage, and planning for major events.
  • Supports the coordination and engagement of volunteers and communicates any concerns to the Director or Volunteer Coordinator.
  • Shops for Life Enrichment supplies and assists with financial tracking (credit card statements, invoices, check requests) as needed.
  • Escorts residents to and from activities; ensures proper use of wheelchairs and safety equipment.
  • Maintains birthday lists and tracks resident participation.
  • Joins in performing household tasks, supporting meal services as needed, and modeling a homelike environment.
  • Responds to residents call bells and alarms within scope of role.
  • Complies with all infection control, safety, and HIPAA policies and procedures.
