About The Position

The San Francisco Campus for Jewish Living (SFCJL), located in San Francisco's Excelsior neighborhood, is a continuum of care provider that includes luxurious assisted living and memory care services, long-term skilled nursing, rehabilitation, and an acute psychiatric unit for older adults. They foster an inclusive culture and welcome employees from all backgrounds who share their dedication to serving older adults with respect. This is an on-call role, working as needed.

Requirements

  • Bachelor’s degree in recreational therapy, psychology, social work, healthcare, recreation administration, or a related field preferred.
  • At least two years of experience working with older adults in group or long-term care settings.
  • Excellent organizational, communication, and group facilitation skills.
  • Creative, flexible, energetic, and resourceful approach to resident engagement.
  • Sensitivity to a variety of physical, emotional, and cultural needs.
  • Ability to work both independently and as part of an interdisciplinary team.

Responsibilities

  • Plan and lead meaningful programs that enhance residents' quality of life through group, one-on-one, and self-directed activities.
  • Promote resident engagement, independence, and emotional well-being.
  • Complete all required documentation, including Life Enrichment Initial Assessments, MDS, progress notes, change of condition updates, and attendance records.
  • Develop and maintain person-centered care plans.
  • Collaborate with Nursing, Social Services, Dietary, Rehabilitation, Volunteers, and external case managers to ensure resident safety and holistic care.
  • Keep activity boards and calendars updated daily.
  • Distribute resident mail, menus, activity schedules, and refreshments.
  • Supervise student interns and volunteers involved in activity programming.
  • Communicate with residents’ family members regarding participation and well-being.
  • Provide opportunities for social interaction and relationship-building among residents.
  • Create a positive, caring, and stimulating environment that supports resident well-being.
  • Re-motivate residents to engage in life through physical, mental, and social activities.
  • Tailor activities to accommodate diverse needs and abilities of residents.
  • Provide individual engagement and support for residents who decline or are unable to attend group activities.
  • Offer emotional support and companionship, especially during times of personal stress or crisis.
  • Perform other duties as assigned.
