About The Position

The Licensed Vocational/Practical Nurse performs nursing care for patients of all ages in their place of residence, coordinates care with the Interdisciplinary Group/Team, patient/family, and referring agency, and contributes to the coordination of care. The Licensed Vocational/Practical Nurse reports to the Hospice Administrator.

Requirements

Two (2) years experience as a Licensed Vocational/Practical Nurse in a clinical care setting and one (1) year full time hospice.
Graduate of an accredited Diploma or Associate School of Nursing.
Current State license as a Licensed Vocational/Practical Nurse.
Current driver’s license.
Nursing skills as defined as generally accepted standards of practice.
Knowledge of state and federal regulations.
Good interpersonal skills.
Reliable transportation and valid auto liability insurance.
Prolonged standing and walking required, with ability to lift up to 50lbs and move patients.
Requires eye-hand coordination and manual dexterity.
Ability to communicate with patients, families, physicians, co-workers, and visitors to exchange accurate information regarding patient condition and health status.
Ability to exchange and express information utilizing language and communicate information effectively.
Ability to hear the nature of sounds.
Ability to give full attention to what other people are saying, take time to understand the points being made, ask questions as appropriate, and not interrupt at inappropriate times.
Ability to view tablet, Electronic Health Record, and review, assess, record or type data quickly and accurately.
Ability to make independent judgments and decisions is required.
Ability to determine resources needed to provide quality patient care.
Ability to travel to patient homes and office/support center locations to deliver care or for education purposes.

Responsibilities

Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation clinical record, team report, and evaluations.
Participates in planning and implementing care in conjunction with the RN, delivers and documents care based on the Interdisciplinary plan of care.
Provides nursing care utilizing infection control measures (OSHA).
Assures the continuity of care through delivery of quality patient care.
Works with the RN and the Interdisciplinary Group/Team to ensure patient/family needs are met.
Communicates with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
Timely submission of all required paperwork.
Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency.
Participates in the Hospice’s QAPI program.
Promotes the Hospice’s philosophy and administrative policies.
Meets productivity standards.
Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
Provides effective communication to patients, their family members, team members, and other health care professionals.
