About The Position
Willow Creek Healthcare Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the AM shift. Requirements
- Active California LVN license in good standing
- Current CPR certification
- Strong communication, organization, and time-management skills
- Compassionate, reliable, and team-oriented
Nice To Haves
- Skilled nursing or long-term care experience preferred
- Familiarity with EMR systems (PointClickCare preferred)
Responsibilities
- Provide direct nursing care in accordance with resident care plans and physician orders
- Administer medications and treatments accurately and timely
- Monitor residents for changes in condition and promptly report findings
- Document care, observations, and changes in condition in the EMR system
- Assist with admissions, discharges, and transfers as needed
- Collaborate with CNAs and interdisciplinary team members to deliver resident-centered care
- Maintain a clean, safe, and supportive environment
- Follow all infection control, safety, and confidentiality protocols
