About The Position

Under the direction of the registered nurse or other licensed independent practitioner, and within the LVN scope of practice, the purpose of this position is to protect and promote health, prevent illness and injury, and alleviate suffering by providing holistic care to patients and families served. This role is within the SRCC Medical Oncology-Visalia department and involves a Day - 10 Hour Shift.

Requirements

  • CA Licensed Vocational Nurse
  • BLS
  • Intravenous therapy certification and Blood withdrawal certification within six months of hire/transfer.
  • Requires successful completion of the Language Resource Assistant (LRA) test at a General level (if selected).
  • For SRCC and Float pool: requires Intravenous therapy certification and Blood withdrawal certification upon hire/transfer.
  • For Sub Acute: requires six months experience within the past two years working in a general acute care facility, or an acquired equivalent experience appropriate to the type of subacute patient the facility provides care for.
  • For Chronic Disease Management Center: requires bilingual in Spanish.
  • For Clinical Education/DSD: requires a minimum of one year experience in long term care.
  • Prefer current State designation as a DSD.
  • For Kaweah Health Exeter Health Clinic: prefer bilingual in Spanish.

Responsibilities

  • Protect and promote health, prevent illness and injury, and alleviate suffering by providing holistic care to patients and families served.
  • Use the results of quality of care/performance improvement activities to initiate changes in nursing practice.
  • Collect clinical data for the RN to monitor quality and effectiveness of nursing care.
  • Identify aspects of care that are important for quality monitoring.
  • Act as a patient/resident advocate under the direction of the RN.
  • Collaborate and communicate with the patient/resident, family, and other health care providers.
  • Make referrals, including provisions for continuity of care, as needed.
  • Assist in development and implementation of policies, procedures, and practice guidelines to improve quality of care and nursing practice.
  • Participate on interdisciplinary teams.
  • Ensure actions on behalf of patients/residents are determined in an ethical manner.
  • Maintain patient confidentiality and privacy within legal and regulatory parameters.
  • Deliver care in a nonjudgmental and nondiscriminatory manner that preserves patient autonomy, dignity and rights.
  • Set up procedure areas appropriately based on patient visit needs.
  • Perform initial nursing intake.
  • Prepare patients for examination reviewing current medications and documents in the medical record.
  • Provide basic patient care and treatments such as taking vital signs, phlebotomy and specimen collection.
  • Collaborate with the interdisciplinary team on patient’s plan of care.
  • Access or deaccess patients’ Port-a-Cath for treatment.
  • Administer oncological medications and provide intravenous hydration.
  • Demonstrate the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
  • Be knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs.
  • Effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
  • Perform other duties as assigned.
