Licensed Vocational Nurse
About The Position
The Licensed Vocational Nurse (LVN) provides direct nursing care under the supervision of a Registered Nurse and ensures physician orders are implemented, medical appointments are coordinated, and continuity of care is maintained for individuals with complex medical needs. LVNs may work on the floor with Direct Support Professionals (DSPs) as part of the staffing ratio when needed and assist in training DSP staff on medical needs and care plans. The role is based in a structured, team-based environment that supports nurses in delivering high-quality care. The scheduled shift is Monday through Friday, 7 a.m. to 3 p.m. Compensation is $31.11–$33.15 per hour, based on experience.
Requirements
- Valid California LVN license.
- Must be 18 years of age or older.
- Valid driver’s license required.

Responsibilities
- Administer medications and treatments as ordered.
- Monitor vital signs, blood glucose, seizure activity, skin integrity, and overall health status.
- Provide skilled nursing care within the LVN scope of practice.
- Schedule and coordinate routine and specialty medical appointments.
- Ensure medical documentation accompanies residents to appointments.
- Receive, process, and implement physician orders promptly and accurately.
- Track follow-up appointments, labs, and referrals to ensure completion.
- Maintain accurate medication administration records (MARs) and clinical documentation.
- Report changes in condition to the RN and medical providers.
- Participate in interdisciplinary team meetings.
- Respond appropriately to medical emergencies.
- Work on the floor with DSPs as part of the staffing ratio as needed.
- Assist in training DSP staff on medical needs and care plans.
