About The Position

Mission Valley Post Acute is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a full-time basis. As an LVN, you will play an essential role in providing exceptional nursing care, supporting resident wellness, and collaborating with an interdisciplinary team to ensure the highest quality of care. Requirements

  • Current California Licensed Vocational Nurse (LVN) license in good standing.
  • Graduate of an accredited vocational nursing program.
  • Current CPR/BLS certification.
  • Strong clinical assessment, communication, and documentation skills.
  • Passion for providing high-quality, compassionate patient care.

Nice To Haves

  • Previous experience in a skilled nursing, rehabilitation, or post-acute care setting is preferred.

Responsibilities

  • Provide skilled nursing care in accordance with physician orders and individualized care plans.
  • Administer medications, treatments, and other nursing procedures safely and accurately.
  • Monitor residents' conditions and promptly report changes to the Registered Nurse and attending physician.
  • Assist with admissions, discharges, and transfers.
  • Complete accurate and timely nursing documentation in accordance with facility policies and state and federal regulations.
  • Maintain electronic medical records and medication administration records (MARs).
  • Ensure compliance with infection prevention, safety, and quality assurance standards.
  • Work closely with physicians, Registered Nurses, therapists, and other healthcare professionals to coordinate resident care.
  • Communicate effectively with residents and their families regarding care plans and progress.
  • Participate in interdisciplinary team meetings and resident care conferences.
  • Follow infection control procedures and safety protocols.
  • Respond promptly to resident needs and emergency situations.
  • Help maintain a clean, organized, and safe clinical environment.
  • Provide compassionate, resident-centered care while preserving dignity and respect.
  • Supervise and support Certified Nursing Assistants (CNAs) as assigned.
  • Maintain resident confidentiality in accordance with HIPAA regulations.
  • Contribute to a positive, collaborative workplace culture.
