About The Position

The LVN provides direct nursing care under the supervision of a Registered Nurse and ensures implementation of physician orders, coordination of medical appointments, and continuity of care for individuals with complex medical needs. At Merakey our LVN's are expected to work the floor with DSPs as a part of ratio as needed, they also aid in training the DSP staff on various medical needs and care plans. This is a structured, team-based environment where nurses are supported and empowered to deliver high-quality care.

Requirements

• Valid California LVN license
• Must be 18 years of age or older
• Valid driver’s license required
• Must be willing to assist with ADLs and other household duties as assigned

Nice To Haves

• Experience working with individuals with developmental disabilities preferred

Responsibilities

• Administer medications and treatments as ordered
• Monitor vital signs, blood glucose, seizure activity, skin integrity, and overall health status
• Provide skilled nursing care within LVN scope of practice
• Schedule and coordinate routine and specialty medical appointments
• Ensure medical documentation accompanies residents to appointments
• Receive, process, and implement physician orders promptly and accurately
• Track follow-up appointments, labs, and referrals to ensure completion
• Maintain accurate MARs and clinical documentation
• Report changes in condition to RN and medical providers
• Participate in interdisciplinary team meetings
• Respond appropriately to medical emergencies
• Work the floor with DSPs as a part of ratio as needed
• Aid in training the DSP staff on various medical needs and care plans
