About The Position

The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) provides direct nursing care and clinical support to individuals receiving services, under the supervision of a Registered Nurse and/or licensed physician, in accordance with California Board of Vocational Nursing and Psychiatric Technicians (BVNPT) regulations. This role includes skilled nursing care, health monitoring, and specialized experience supporting individuals with Type 1 and Type 2 Diabetes, ensuring safe, compliant, and person-centered care.

Requirements

  • Current California LVN/LPN license in good standing
  • LVN or LPN CA license
  • valid driver’s license
  • 1 year experience working with individuals with developmental disabilities and/or mental health issues
  • Knowledge of medication administration, chronic disease management, and supportive behavioral health environments

Nice To Haves

  • Demonstrated experience supporting individuals with Type 1 and Type 2 Diabetes preferred
  • experience working in a residential environment preferred

Responsibilities

  • Provides direct nursing care and ongoing health monitoring under appropriate clinical supervision, in compliance with BVNPT standards and physician orders
  • Delivers diabetes-related nursing care including blood glucose monitoring, insulin administration, oral medication administration, symptom recognition, and documentation for individuals with Type 1 and Type 2 Diabetes Mellitus
  • Implements individualized care plans, treatments, and medical protocols as ordered by licensed providers
  • Coordinates medical appointments, specialty care, and follow‑up services for individuals receiving care
  • Provides on-site clinical support, assessments, and interventions as within LVN scope of practice
  • Collaborates closely with the Program Director, Administrator, and interdisciplinary team to ensure health, safety, and regulatory compliance
  • Educates and supports direct care staff on medical protocols, medication administration, diabetes management practices, and emergency response procedures
  • Communicates appropriately with family members, guardians, healthcare providers, and external stakeholders as needed
  • Represents the organization at medical appointments, interdisciplinary meetings, and external conferences as assigned
  • Maintains knowledge of program regulations and assists with preparation for licensure reviews, audits, and surveys
  • Participates in on-call rotation to support medical needs outside of regular business hours
  • Completes accurate, timely documentation in accordance with organizational and regulatory standards
  • Performs additional related duties as assigned
